Pelviureteric junction obstruction (PUJ) is a condition where there is a blockage or narrowing of the area where the kidney connects to the ureter (the tube that carries urine from the kidney to the bladder). This can cause urine to build up in the kidney, which can lead to pain, infection, and kidney damage if left untreated.
PUJ obstruction is most commonly diagnosed in infants and young children. With routine use of ultrasound during pregnancy, PUJ obstruction may present as a swollen kidney detected on prenatal ultrasound. Few children may present with pain or lump in abdomen or may be incidentally diagnosed during an ultrasound.
If you suspect your child may have PUJ obstruction, it is important to seek medical attention as soon as possible. With prompt diagnosis and appropriate treatment, most children with PUJ obstruction can have normal kidney function and lead healthy, active lives
Treatment for PUJ obstruction may be wait and watch or surgery. Consult with your child surgeon to understand what is best for your child.
Surgery related
Surgery if performed, involves surgery to remove the blockage or widen the narrowed area. In some cases, a stent (a small tube) may be inserted to help keep the ureter open. The surgery can be perforemd by open surgery or laparoscopy (keyhole surgery).After surgery, it is important for the child to have regular follow-up appointments to monitor kidney function and ensure that the condition does not recur (can happen in upto 5% children).
Follow up
The long-term outlook for individuals who undergo PUJO surgery is good, with most experiencing relief of symptoms and improved kidney function following the procedure. However, the specific long-term outcome will depend on various factors, including the severity of the obstruction, the individual's overall health, and the success of the surgical procedure.
In some cases, individuals may experience complications after PUJO surgery, such as infection or bleeding. However, these complications are rare and can often be successfully managed with appropriate medical care.
